Paolo Carpaneto has always been the type of guy who was interested in getting in shape, but he just didn’t know where to begin. Like many others in his position, he didn’t come from a background of weight lifting or practice gym training and only briefly sports in his youth.

“I always wanted to start any activity to get back in shape, but I didn’t take it seriously,” Carpaneto said. “I didn’t have the support that I needed to continue and progress on my goals.”

But in 2020, when his weight started to balloon and his daily activities began to become hindered, Carpaneto knew that he needed to act, and act now.

“At the time I was 227 pounds. Climbing the stairs, lacing my shoes, and finding clothes were all issues,” he said. “I think this was my ‘low point’, and I said to myself: it’s now or never.”

Many people have experienced the type of low-point that Carpaneto is describing, and it can be made even more tough by not having someone in your corner to motivate you to get your life back on track. This is what ultimately spurred his decision to contact Ultimate Performance Singapore.

It’s important to recognize that everyone feels a little overwhelmed when starting a new training plan, but the key is to stick with it. With the help of his trainer, Owen, Carpaneto was able to remain consistent in the gym and add strength fast.

“I usually have three sessions with Owen every week and one or two running /jogging alone,” he said. “I had several weight programs too. The first was to learn the basic movement, then I had the 5×5 program and then high repetition. We gave them funny names such as Arnold Mode and TROY, but all of them are full body workout.”

While Carpaneto found success in his new strength training routines, he also needed to get his diet on track too. This is often the step that people neglect when starting a new workout plan. As they say, six-packs are made in the kitchen, not the gym.

Prior to his transition, he would usually start the day with black coffee and finish it off with high- calorie and carb foods for lunch and dinner. This was something his trainer knew Carpaneto immediately needed to stop.

“It was not easy. Owen put a lot of effort on teaching me how to track the food, the calories and the macros,” Carpaneto said. “He started from the basics. He gave me four meals per day and suggested a basic menu with plenty of options. He also sent me a lot of podcasts and articles that explained the importance of nutrition and being consistent to see results.”

However, not all of the changes he’s experienced are purely physical. Carpaneto said that his weight-loss transformation has helped him feel more confident and full of energy—even giving him what he describes as a “Navy Seal mindset.” His new life has also spurred his love for a long-lost sport—running.

“Before the transition, I wasn’t able anymore to run for more than 100 meters,” he said. “This was something that made me depressed. Now, I can perform a 5K in a reasonable time. I ran in a competition here in Singapore, and I did my 5K in little more than 30 minutes. Not bad for +40-year- old at his first competition.”

Despite all the progress Carpaneto has made during his weight-loss transformation, he’s not done achieving his goals yet.

“I’m not yet finished,” he said. “At the moment, I have one session every week with Owen, and we are working to build more muscle in a long term project. This will challenge me more on staying consistent and try to put in place what I learned until now.”

“It has destroyed my life,” a car crash survivor says of changes to Michigan’s car insurance law

Brian Woodward is the kind of success story Michigan’s old auto no-fault law was quite literally designed for.

After a car crash left him a quadriplegic at age 24, he didn’t have to sue to get his care paid for, or spend the rest of his life in a nursing home. That’s often the fate of severely injured people in other states.

Instead, he went to college, with the help of one-on-one skilled care around the clock. He graduated magna cum laude from Northwood University with a Bachelors degree in business and computer science.

“I ended up being a contractor to one of the Big Three automakers,” Woodward said. “In that time, I paid taxes. I earned enough money to buy my own house. I was active in my community.”

Woodward tutored and mentored children and other quadriplegic crash survivors. Because he earned his own income, he could help neighbors in need with groceries. He paid teenagers in the community to mow vacant lots.

“I sang in the choir in my church,” he said, noting that having a deep bass voice enabled him to hold his own in performances, despite not being able to use the muscles in his chest like other singers do.

One of his caregivers even took him hunting, fishing, and boating. It was a full, rich life.

It actually has destroyed my life. The life as I knew it was completely gone in a moment.

But “on July 2, the world came crashing down,” he said of the day he lost his care at home. Michigan’s new insurance law “has actually destroyed my life.”

July 1, 2021 was the date set by Michigan’s new no-fault law when insurance companies were permitted to begin slashing reimbursements for the care of catastrophically injured car crash survivors by nearly half.

Brian Woodward worked as a contractor for a Big Three automaker for 30 years before losing his caregivers

Woodward’s agency said they couldn’t afford to take care of him anymore because of it. The cuts made their business unsustainable. One of Woodward’s caregivers had been with him for 20 years, another for 26 years.

“Those are like your best friends,” he said. “You develop your own little family with the people that take care of you. And it was very heartbreaking and heart-wrenching to lose them like that.”

Without caregivers, he also lost his job of 30 years.

Since the summer, he’s lived in a string of understaffed nursing homes. Quadriplegic people need vigilant care to stave off infections, pressure sores, blood pressure spikes, and overheating.

“I ended up going to the hospital at least six or seven times throughout this ordeal,” he said. “Several of them were life-threatening. I had sepsis twice.”

The facility Woodward is in now — Special Tree Neurocare Center — is better than the others, he said. But he’s still scared. He used to have someone with him at all times if something went wrong. If something were to go wrong now, when he’s in his room alone, he wouldn’t be able to push a call button to get help.

Woodward is praying that the politicians in Lansing do something before it’s too late. People have already died after losing care. He’s afraid he could be next.

So far, the new insurance law has driven 96 companies out of the business of caring for catastrophically injured car crash survivors, with a loss of more than 3,000 jobs, according to an independent study by the Michigan Public Health Institute..

More than 1,500 survivors have lost their care.

“You just can’t have folks who can’t move — who are quadriplegics — and just leave them in an empty building. Something’s gonna have to give.”

“I never thought it would get this far,” said Special Tree Neurocare Center CEO Joe Richert. He met with me the same day he had to lay off 30 staff members.

Richert says the company has already gone through $7 million of its reserves, and there’s not much left.

“Part of the problem is the insurance companies aren’t even following the bad law,” he said. The law allows insurance companies to impose a 45% cut to the reimbursement the company was receiving for services in 2019. “But they’re just not paying anything.”

Richert said Special Tree has received no reimbursement for six months of care for several high-needs patients. Complaints to the Department of Financial and Insurance Services were closed without explanation, he said, after DIFS said it had received a response from the insurance company in the case.

Richert calls the new insurance law “crazy,” because it permits one highly profitable industry — car insurance — to drive another one that’s far less profitable out of existence. Companies like his are closing all around him, and vulnerable patients are losing care, even dying.

“It’s despicable,” he said. “It’s not exactly organized, but it’s essentially all coming together in a plan to destroy us. It’s evil — at least an evil outcome, even if their intentions aren’t evil.”

Richert says Governor Gretchen Whitmer and a few key Republican leaders could put a stop to the debacle. There’s nearly $27 billion in the state fund that was set up to care for catastrophically injured car crash survivors.

But instead, the fund’s surplus is being used for a campaign stunt, he says. Governor Whitmer asked insurance companies that control the catastrophic claims fund to approve a refund check this spring, “so the governor can stay on her victory lap and give back $400 per vehicle before the election,” Richert says.

The governor’s office did not respond to a request for comment.

The Insurance Alliance of Michigan refuses to grant interview requests from Michigan Radio about the crisis. But in periodic press releases, they steadfastly defend the new law, saying it’s working as intended.

In its most recent press release, IAM said car crash survivors were actually getting too much care.

“Overutilization can greatly harm the health and well-being of a patient,” the IAM statement claimed.

Providers say the complete collapse of the industry that cares for car crash survivors will come within months, as companies like Special Tree finally exhaust their reserves or loans.

Richert has no idea where his patients will go if he’s forced to close.

“You just can’t have folks who can’t move — who are quadriplegics — and just leave them in an empty building. Something’s gonna have to give.”

Survivors, their loved ones, and care providers have pinned their hopes on a new bill with fairly strong bipartisan support that was introduced by state Rep. Phil Green (R-Millington).

The bill would significantly boost payments for care.

Some advocates say the real solution is to repeal the part of the new law that allows insurance companies to cut payments for care retroactively for survivors, many of whom have depended on that care for decades, like Brian Woodward has.

But time is running out.

To any young person reading this: If, in a seeming-romantic relationship, you see signs of mental, emotional and most worrisome, physical abuse, RUN!

Dear Ellie: I was 19 when I met my future husband. He was everything I wasn’t — self-confident and cool. He was 24, claiming to show me “what life is all about.”

I was smitten, but my father said I was too young for such intense dating. So, I started sneaking out before my father arrived home from work… cheerily telling my mother I was going to a girlfriend’s house, a movie, or studying with someone to get into a college course.

The lies worked. Six months later, I was engaged with a modest ring on my finger and beaming happily. My father said he “hoped it’ll be alright.” My fiancée said we must marry immediately so my father couldn’t break us up.

Three months later, I was back home with a black eye.

I lied that I’d bumped into a door when I awoke in the night, so my soon-to-be ex wouldn’t be charged with physical assault. My father feared that otherwise, he’d stalk me forever after.

A lawyer helped me get a quick divorce and I moved far east across the country to relatives I’d never met before. I wasn’t easily traced.

Change is inevitable, especially when it comes to business. In the course of an individual’s professional evolution, it sometimes makes sense to embark upon a big change — like spinning off to launch an independent company or taking on an executive position at a new company.

When making a significant change, it’s vital to keep clients, contacts and colleagues well-informed in order to maintain key relationships and business.

1. Understand your former contract and limitations

Prior to making a move, take a close look at any employment agreements and restrictions to which you might be obligated. 2. Prepare, prepare, prepare

  • They’re paying more attention to their appearance

Their Routine Has Changed

Bolu Oladini, founder of life and relationship coaching service Confluence, said taking up a new activity that seems out of character is suspicious, especially if it’s time-consuming. “The real key is the sudden nature,” he told Newsweek. “They were never into tennis—and now they’re playing two to three times a week.”

If your partner seems overly interested in where you’re going to be and when, they could be trying to suss out when you’ll be busy so they can see their lover.

You Can’t Reach Them

Missing a call verbally is no big deal, but becoming virtually unreachable could mean your significant other is being unfaithful—especially if they’re not responding at times they usually would. Changing the technology they use to communicate is also a bad sign. Whether they’ve bought a second phone, changed their passwords or stopped using shared devices, they may be trying to hide something.

Paul Keable, chief strategy officer at Ashley Madison, told Newsweek: “Cyber ​​affairs saw a rise at the start of the pandemic which has continued to increase in 2022. A quarter of members [are] carrying out their infidelity this way.”

Gone from having sex once a week to once a day? Or perhaps you’ve stopped having sex altogether?

Cable said: “[The] Most obvious hints would be incorporating new moves in the bedroom, or perhaps losing interest in sex altogether.”

If your spouse’s sex drive has decreased, it could be because they’re doing it elsewhere, but it’s also common for unfaithful partners to want more sex. If their libido has suddenly increased, they may be trying to throw you off the scent.

Oladini added that people having affairs often seemed distracted during sex with their partner. “They may emotionally not be there or just going through the motions.”

They’re Paying More Attention to Their Appearance

A snazzy haircut or new devotion to the gym could mean your partner has someone new to impress.

“Increased confidence and putting more effort into their physical appearance like working out more, dressing better or enhanced grooming habits” might all raise your suspicions, according to Keable.

“Nearly 80 percent of [Ashley Madison users] consider their affairs to be a form of self-care,” he said.

If you suspect your partner’s glow-up isn’t for your benefit—or their own—ask yourself if there are any other signs that they’re seeing someone else.

They’re Spending More Money

Nicole Reid, a private investigator based in the UK, told Newsweek that financial inconsistencies are often the first clue that something is amiss.

“Clients will call us when they have seen a booking for a hotel for two people or restaurant bookings, or [have] even found a receipt [when] they have said they are going on a work trip,” she said.

If there are unexpected charges on your joint account or you’ve found a credit card statement that doesn’t make sense, your partner could be splashing out on their side piece.

You Keep Catching Them in Lies

You have good reason to be suspicious if your partner is telling you stories that don’t add up, isn’t where they say they’re going to be, or gets defensive when you ask about their day.

“If they’re making up excuses or their story changes, that’s a big sign,” said Oladini.

Unfaithful people often attempt to gaslight their partner to cover their tracks, according to Reid, managing director of Corporate Investigations UK. If your partner is calling you jealous or unstable, or accusing you of having an affair, they could be trying to shift the focus from their own activities. They “blame their partner and accuse them of cheating, when in fact it’s them,” she said.

Gaslighting makes it difficult to trust your gut, but Reid recommends doing so. More often than not, her clients’ intuition is spot on, she said. “Most occasions, our clients are right and we catch [their partner] cheating.”

Children from historically marginalized groups or poor backgrounds are less likely to survive neuroblastoma

Children with high-risk neuroblastoma had worse outcomes if they were from certain racial/ethnic groups or were on public rather than private insurance, despite being treated in clinical trials with standardized protocols, according to a study led by investigators from Dana-Farber/Boston Children’s Cancer and Blood Disorders Center.

The study shows that young patients from historically marginalized populations or from lower-income backgrounds had poorer five-year survival rates even when they were assigned to receive uniform initial treatment after diagnosis with high-risk neuroblastoma.

These findings recapitulate what we have known for decades at the population level-;children from historically marginalized groups are less likely to survive their cancer. They add an essential next layer to our understanding of racial and ethnic disparities in childhood cancer, and that is enrolled on clinical trials is not enough to achieve racial and ethnic equity in survival.”

Umaretiya is presenting the study results at the American Society of Clinical Oncology (ASCO) Annual Meeting, being held June 3-7, 2022, and the study is included in the ASCO press program.

“Clinical trials representing highly standardized care – yet even when receiving care on clinical trials, children with high-risk neuroblastoma do not experience the same outcomes based on their race, ethnicity, and whether they live in poverty,” said Umaretiya, lead author of the study. “This is key, because thus far attention has been paid to getting historically marginalized groups to trials with the assumption that this will reduce survival disparities, but our data suggest that in pediatrics, trial-enrollment is a first step, but clearly not a sufficient one.”

Senior author is Kira Bona, MD, MPH, a pediatric oncologist at Dana-Farber/Boston Children’s with research focused on identifying poverty-associated outcome disparities in childhood cancer and developing interventions to mitigate those disparities.

Bona notes, “That stark racial/ethnic disparities in survival persisting clinical trial participation makes it crystal clear that pediatric oncology trials must incorporate health equity interventions. If a new gene mutation were found to increase risk for trial-enrolled patients, pediatric oncology would not hesitate to begin intervening.

The study looked at outcomes in 696 children enrolled in three Children’s Oncology Group (COG) clinical trials of treatment for high-risk neuroblastoma. Neuroblastoma is a type of cancer that forms in nerve tissue. It frequently begins in one of the adrenal glands but can also originate in the neck, chest, abdomen, or spine. High-risk disease is defined by age, how widely the disease has spread, and biologic characteristics of the cancer cells. The prognosis for long-term survival remains challenging. Treatment is usually an intensive combination of chemotherapy, surgery, stem cell transplantation, radiation, and immunotherapy.

Of the 696 patients in the COG trials, 11% were Hispanic, 16% were Black non-Hispanic, 4% were other non-Hispanic, and 69% were white non-Hispanic. One-third of the children were poverty household-exposed (covered by public insurance); 26% were exposed to neighborhood level poverty (living in a high-poverty ZIP code defined by 20% or more of the population living below the federal poverty line).

The five-year overall survival rate varied by race/ethnicity (47% for Hispanic children; 50% for other non-Hispanic children; 61% for white non-Hispanic children; and 63% for Black non-Hispanic children.) after adjusting For disease-associated factors, Hispanic children were 1.8 times more likely to die and other non-Hispanic patients were 1.5 times more likely to die than white non-Hispanic children.

Patients who had only public insurance (a proxy for household poverty) had a 53% five-year survival rate compared to 63% for others. The survival rate was also lower – 54% — in children living in neighborhood level poverty compared with 62% for others.

“A huge strength of the way that this dataset was created is that we have the ability to look at potential mechanisms that may explain these survival disparities,” said Umaretiya. “For the first time, we will be able to ask whether certain groups experienced delays in therapy or were more likely to stop participating in trials perhaps because of competing family needs secondary to poverty. Most importantly, we will be able to start to look at what happens after relapse – a time when we know treatment becomes less standardized, which may increase the chance that a racial, ethnic, or socioeconomic privilege helps some families access life-extending therapy for their children while others are less able to. relapse will be essential to guiding interventions to improve disparities and we are excited to take this on next.”
